Pos-_Infographic (2)The mission of San Ysidro Health Center’s HIV Primary Care Program is to offer comprehensive care to enhance the health and quality of life of patients with HIV/AIDS. HIV primary care services offer diagnosis and treatment of HIV, treatment of opportunistic infections and other complications, referrals for specialty care, nutritional assessments, and referrals to prevention, mental health counseling, and other support services.

The HIV Primary Care team consists of the following members:

HIV Clinical Providers

Physicians from UCSD’s Owen Clinic, who specialize in HIV, offer primary medical care at SYHC’s main health center site, Monday through Friday.

HIV Nurse Case Manager

The HIV Nurse Case Manager coordinates health and social support services according to individual care plans and assists with referrals to dental care, housing, transportation, as well as other support services. In addition, serves as a liaison to coordinate communication between the patient and medical providers.

Treatment Adherence Counselor

The Treatment Adherence Counselor provides general information on HIV medications, helps patients comply with the treatment plan established by the physician and works with each patient on the development of an individual treatment plan that includes scheduling, dosing, and effective side effect management.

Medical Assistant

The Medical Assistant is responsible for taking vital signs, making appointments and referrals, lab work orders and translation services.
